The director of Vermont’s Blueprint for Health will leave state government at the end of July.

Dr. Craig Jones has served as the chief of the Blueprint for Health within the Department of Vermont Health Access since August 2007, according to the Human Resources Department. He made $154,647 in 2014.

The spokesperson for the Department of Vermont Health Access did not return inquiries for comment. Jones was not available Wednesday afternoon.

The Blueprint for Health is a health care reform program that encourages doctors in the state to follow an integrated model to treat patients holistically.

The patients establish “medical homes” with their primary care physicians and may get free access to someone who will coordinate their care and provide services such as counseling, substance abuse treatment and health coaching.

According to the program’s website, “The Blueprint also helps design innovative interventions, like Support and Services at Home for elders and Hub & Spoke Medication Assisted Therapy for individuals battling opioid addiction.”

“All Blueprint work is closely integrated with health and human service organizations through Community Collaboratives that guide care delivery and payment reforms at the local level,” the website says.
